Traditionally the diagnosis of male infertility relies upon microscopic assessment and biochemical assay to determine human sperm quality. Most important parameters given are the concentration, motility and morphology of sperm in the ejaculate.Simplest test for the female partner is a transvaginal scan on her 12th day of her menstrual cycle. This is a technique used to view the pelvis and any other pathology. Ultra sound is an imaging technique using sound waves which are transmitted through a transducer (probe).<img src="http://www.theembryologist.com/images/naturalCycle.jpg" alt="" class="fr-fic fr-dii fr-draggable " style="" />In fertility practice scans are done with a transducer covered with a sterile condom placed internally which clearly shows the womb and the ovaries. It is a painless procedure, performed with an empty bladder. On the 12th day of the cycle there should be a dominant follicle with a diameter of 18 to 20 mm, which carries a mature egg inside. If the scan does not show a dominant follicle female partner should be given fertility medicine in her next cycle. Rescan is performed in her next cycle 12th day. When the follicle is around 20mm it ruptures to release its egg (ovulation).In a natural 28 day menstrual cycle fertility period is between 12th to 16th days of the cycle. Intercourse in this particular period ejaculates sperms into the vagina. Then out of millions of sperm that is produced at one ejaculation, the most healthy ones swim up the cervix and into the womb cavity. At this time if ovulation takes place and egg is released from the ovary, the sperm fertilizes the egg in the fallopian tube. Out of millions of sperm only one fertilizes the egg.After 18hours from fertilization paternal and maternal genes get together and form a zygote, which divides and form a 2 cell embryo in 24hrs. In 44hrs the embryo becomes 4 cells and in 60-70hr it divides up to 8 cell embryo. This 3 days process still happens in the fallopian tube where the embryo rolls towards the uterine cavity. On the 4th day it divides more and becomes compacted. This is known as morula. On the fifth day the embryo rolls into the uterine (womb) cavity and complete the compaction and starts to cavitate. This is known as the blastocyst.The blastcyst has two components: Inner cell mass(ICM) which becomes the baby and Trophoblastic epithelium(TE) which covers the blastocyst becomes the placenta. On the fifth or sixth day after fertilization the blastocyst is embedded in the womb. This process is called implantation.
