SLT shall deal or liaise with the customer with regard to any claims</p><p> made by the customer.</p><p><span style="color: #0066cc">• </span>All claims will be paid after consultation with SLT. </p><p><span style="color: #0066cc">• </span>If the cause of death of the customer is due to an illness, only the claims arising </p><p> after a lapse of sixty (60) days from the date of payment of the first premium shall </p><p> be considered for settlement.</p><p><span style="color: #0066cc">• </span>All claims must be submitted within 3 months of Death or Total and Permanent Disability due to an accident. </p><p><span style="color: #0066cc">• </span>No claim arising due to a pre-existing illness (i.e. due to an illness which was</p><p> contracted by the life assured, prior to enrolment) will not be paid.</p><p><span style="color: #0066cc">• </span>And according to the Terms and Conditions given in the Insurance ceritificate</p><p></p><p><strong>Terms and conditions</strong></p><p><span style="color: #0066cc">• </span>If any customer has not settled the payment for three (03) consecutive months, his</p><p> agreement will be terminated.</p><p><span style="color: #0066cc">• </span>Customer can terminate the agreement at any time by calling SLT Call Center on </p><p> 011 2 555555 and making a request.</p><p><span style="color: #0066cc">• </span>Customer has to sign and submit a form of <strong>Declaration of Good Health</strong> to SLT</p><p><span style="color: #0066cc">• </span>When a customer's phone is disconnected then the insurance scheme will also be</p><p> terminated.
