- Supplier registration criteria were unjustifiably relaxed: the requirement that a supplier must have previously provided 500,000 MT of coal with a Gross Calorific Value (GCV) over 5,900 kcal/kg was reduced to just 100,000 MT.
- Out of the 1.87 million MT of coal previously supplied by Trident Chemphar, over 1.57 million MT had a GCV between 4,900 kcal/kg and 5,900 kcal/kg, falling well below the plant's standard of 6,150 kcal/kg.
- Bids for the 2025/2026 season were officially invited on August 18, 2025, but bid documents were issued to Trident Chemphar Limited before they had fully paid their registration fee.
- Quality reports at the loading port were issued by Mitra SK South Africa (Pty) Ltd., an entity lacking the required accreditation. their testing accreditation expired on December 29, 2025, and no renewal certificate was provided even by March 31, 2026. Lanka Coal Company failed to appoint an alternative independent inspector or utilize the contractual option to test an Umpire Sample, resulting in none of the 2025/2026 shiploads meeting the required 6,150 kcal/kg standard.
- To mitigate the ongoing crisis caused by the delays and the 2025 tender cancellation, another emergency Spot Tender was awarded on March 18, 2026, to Taranjot Resource (Pvt) Ltd.—a company previously identified as having supplied substandard coal.
