1.A bank receipt for depositing Rs. 325/- paid to any branch of Peoples Bank to the credit of A/C No: 055-1001-1-0667549, Peoples Bank, Kelaniya Branch.
2.Certified true copies of the results sheets by the Head of the School or a Justice of Peace with the official stamp of the certifying officer for (a) G. C. E. (A/L) 2013 and Z - Score (b) G. C. E. (O/L).
3.If the names on any of the above documents differ from that of application, an affidavit to confirm that they relate to one and the same person.
