ETF claim form

Dear all,

I need a help.
I my office my responsibility is to fill EPF ETF
now I'm struggling to fill a ETF claim form.
in our office no records about etf contributions below 2013 year
I should fill the amount quarterly from year 2009
but no R2 form and C form also misplaced
can anyone help me to fill the ETF form part 2 to be fill by employer , item number 7

Just go to ETF. They will tel you what to do. They have all the information in their servers.
Make sure to bring id copy and claim form.
