if the scanned document is in PDF format, you can use the text selection tool in the PDF reader.
Otherwise you need a good OCR.

You need OCR Software to do this. The best & the Latest is ABBYY FineReader 9.0 (I'm using it) Even you scan to pdf you need the Acrobat Writer software to grab the text or otherwise you have to select it from the text selection tool copy it and past to MS Word or any other format. if you need the OCR DL links please post here
