The name of the song is actually "Love, Me". It's about never ending love.... Nothing can break them apart. In the song the grandpa is explaining this to his grandson. He shows the grandson the note grandma wrote him when they were young, telling him that she'll come no matter what... "If you get there before I do, don't give up on me." The grandson reads the note right before the grandma dies and goes to the church with the grandpa where grandpa tells her the same thing she told him all those years ago... Meaning, if she gets to heaven before he does, not to give up on him, he'll be there....just like she was for him.
