You have to fill a form and submit it to the Department of Immigration Head office stating that you are not a Citizen by birth or you do not intend to get the citizenship of your birth country. They will put a stamp on the back side of the Citizenship Act. Its a mandatory document if you are born outside of Sri Lanka to get the Sri Lankan Passport.</blockquote>
