is there any govt regulation asking the employer to carry out PCR tests daily/weekly? who monitors that this is done by employers without skipping?

I think for BOI projects laydown procedure is there and weekly reporting to BOI is a must. No idea for others

Yes there is, The sample must be 1% of the working staff (Minimum) on a working day. Prior that you need to contact the relevant MOH and get the samples. All the tests are done by the Private Labs known as "Accredited" (Nawaloka, Asiri, Lanka, Durdans).

Not mandatory but it is up to you to decide whether shutting down the whole factory in case or taking precautions to minimise the risk.
