Try uninstalling all controllers (forget about other devices) listed in Device3 Manager under Universal Serial Bus controllers. Reboot...Windows will reinstall the controllers automatically. Try the following sequence.

Start/Run...type chkdsk /r and hit Enter. Type Y in new screen and hit Enter.

    Then uninstall all USB controllers.

Reboot. The chkdsk /r command will execute, upon completion the system will boot into Windows.

    Try this -
    1. Go to "My Computer" and click it, then on the sub-menu click "Manage".
    2. On the left hand side click "Disk Management" under "Storage".
    3. Navigate to your device. When you see it, right click it and select "Format".
WARNING - You will lose all data.
    4. Continue, uncheck "Perform quick format" and press "OK".
    5. If you get an error saying it failed, repeat the process - but this time check "Perform quick format" and press "OK".

    These steps worked for me.
