Laptop is Slow even after formatting... Need HELP!!

I reformatted my laptop because it got super slow(and started checking my Local disk D at startup)
But even after formatting It's still the same (I formatted all the partitions),,, It takes more then 5 mins to on...and about 2 mins to open a file...
I formatted it 3 times.. But it remains as it was..

Can any one please help me on this matter???

Windows reinstalled with full formatting?

Try with some software which can fix the physical errors..formatting wont help
