My concern is not with the act of masturbation. It is with the thoughts we have while masturbating. Many of these thoughts are very impure. We fantasize about other women, sometimes even married women! In my opinion, THAT is where the evil lies, not in the act.

if you not take our religion advise in fun i can tell you the real situation of permission of this act.

Islam teach people to marry in early age (when sexual desire starts) to save people from such type of unnatural acts.The people who has not such resource to marry, islam teach them to remain away yourself from environment and things which increase the sexual desire.
but if it happen due to the environment or through any other matter that a man has come at the stage where if he not commit this sin may be he attracted or inclined to commit zina (fornication), at this stage Islam has given flexibility and permission for this act to that people to commit this small sin instead of committing that big sin zina (fornication).

But if a person that intentionally join an environment that boost sexual desire or has made this act a habit to fulfill his sexual desire that are not allowed in islam.
Quran says.
Successful indeed are the believers. (1)
Those who offer their Salât (prayers) with all solemnity and full submissiveness. (2)
And those who turn away from Al-Laghw (dirty, false, evil vain talk, falsehood, and all that Allâh has forbidden). (3)
And those who pay the Zakât. (4)
And those who guard their chastity (i.e. private parts, from illegal sexual acts). (5)
Except from their wives or that their right hands possess, - for then, they are free from blame; (6)
But whoever seeks beyond that, then those are the transgressors; (7)

How are the nurses, doctors and others going to attend to such patients?

All doctors and other staff in all hospitals in the world disregard this foolish advice by Allah!

And how can a young man bathe an old, feeble man? Maybe his father?
