Briefly this drug belongs to a class called "Checkpoint Inhibitors". Cancer cells usually places a shield around them that blocks T-cells in our immune system, so the T-cells cannot attack them. A Checkpoint Inhibitor removes this shield, and thus the cancer cell gets exposed and thus our immune system destroys them.

Immune Checkpoint Inhibitor therapy isn't new... A decade old. The first FDA approved drug was in 2011, Ipilimumab, the first antibody blocking an immune checkpoint (CTLA4) inhibitor. There are several others eg: Pembrolizumab (Keytruda®) and Nivolumab (Opdivo®), Atezolizumab (Tecentriq®) as well.

Jemperli (Dostarlimab) was invented in 2014. It's a Programmed Death Receptor-1 (PD-1)-blocking antibody that binds to the PD-1 receptor and blocks its interaction with the PD-1 ligands PD-L1 and PD-L2. ( A Ligand is molecule that irreversibly binds to a receiving protein molecule, otherwise known as a receptor)
It was approved for patients who has a biomarker called dMMR. this biomarker is commonly associated with endometrial, colorectal and other gastrointestinal cancers... but also it may be in other solid tumors too.
In normal cells MMR (MisMatch Repair) process corrects DNA replication errors using enzymes. basically it checks the integrity of the DNA strand and fixes any mistakes. For some people who are dMMR (deficient MMR) these enzymes do not function as normal, thus the errors are not corrected leading towards a cancer cell.

Dostarlimab can be used in combination with Niraparib, Carboplatin–Paclitaxel, sometimes with Bevacizumab in advanced cancers. If you check the US databases you could probably find several thousands of cancer trials using these drugs. Also wonder how long and the associated costs it takes to test, develop these drugs.
(These guys aren't as advanced as our local druids who mix thel & pani concoctions )

Monoclonal antibodies aren't cheap. Dostarlimab was given to the test patients every three to six months at a cost of $11,000 each dose. If the drug proves itself many governments will subsidise cost to the patients. Also the Indians will be able to make these at a much lower cost.
Whatever it is, it's good news.

Yes.. Hopefully the Science will advance to that stage one day. Had you read my post now we have reached a stage where the cloaking of the cancer cell can be removed so that the body T-cells fights back. That's what the Checkpoint inhibitor does.
There are several groups around the World studying on why tumor cells sometimes manage to resist being destroyed by both T cells and NK cells. There are about 25,000 genes in a human. It's not easy to figure out what roles these play exactly and how to tweak some to beat cancer.
Latest advances in Epigenetics is on the way too. Now the scientists can change the Epigenome - The chemical tags of the DNA- the proteins that govern a gene’s expression and how it ultimately behaves - without really altering the DNA itself.
