You can request through the online portal or the local MOH office( call them before you visit ). So far, it is issued only for travelling purposes (you have to present a flight ticket). In my experience, requesting with the MOH is much faster than the online portal. Before they enter the details into the certificate, double-check everything as it's pretty difficult to do further changes once the smart certificate is issued.

Site says 'Only one request can be submitted per NIC/Passport' . In my case, I got my first certificate with double dose from MOH. After the booster-shot, I have requested from the online portal mentioning the reason for requesting again. After couple of weeks, they sent me the vaccine certificate with all three doses.
