State governments is responsible to the federal government but at the same time appear to have lot of autonomy with their decision making in day to day function.
There are state elections to elect members to the state parliament and federal elections to elect the PM etc
There's only one Taxation office.
There's federal police as well as state police departments.
The Army, Navy and Air Force is I believe the responsibility of the federal government.
States have their own courts, hospitals etc.,

One Medicare system for the entire country.
I believe that's the system in many countries because it's practical to manage that way compared to all decisions coming from the central government.
