Distance from the hospital to the residence is not considered when doctors are given their posts .So unlike other occupations, doctors wouldn't have the nearest possible working station.( I personally live 35kms away from my working station .It took me more than 3 hours to travel home from the hospital in day time a few months back.I have to take 3 separate buses as well. But 2 of those routes there no functioning public transport in the night.)

Refer to first answer.( For me personally it would take 3000+ for a single threewheel ride. My whole salary would not be adequate do so every day. Anyway I spend more than 35000 on fuel alone even when I use my car)

A reasonable solution if it can cover every shift. For example, ICU shifts start from 8am/2pm/8pm. And with handing over time being not constant everyday leaving time would differ day to day.

Of course I have waited in queues many time and never used the fuel pass to take the priority in the queue. But now the issue is queues extend for several days .Basically you have to wait near the fuel station till a bowser comes and otherwise if you join a queue for a station where the bowser has unloaded you wont be able to get fuel . Its practically impossible for healthcare workers.
So there goes your reasonable solutions. One other thing on-call does not mean that every oncall doctor should be in hospital premises. There is an oncall house office who would be in the ward for 24 hrs. Oncall SHO also would be nearby and would come if necessity arise . There are may be more than 1 oncall SHO depending on the unit and second oncall would come only in case of transfer. And the oncall consultant also has to come if his/her service is required. So the consultant would be oncall on every night for his ward(If he is not available it should be covered by another consultant. Especially in surgical and Gyn/Obs ward consultant can be needed for any time for emergency surgeries. By your logic consultant surgeons and VOGs wont be able to leave the hospital.
I guess you havent experience Sri Lanka outside Colombo. Uber/pick me is limited mainly to city areas. ETU duties and ICU duties also equally mentally exhausting similar to surgeries.

මගෙත් වයිෆ් නර්ස් කෙනෙක්.මේ වගේමයි හොස්පිටල් එකට කිලෝමීටර් 35ක් දුරින් ඉන්නේ.බස් 2කක යන්න වෙනවා. මම හැමදාම දුවවත් ස්කෝලෙට දාගෙන වය්රිෆ් වත් වැඩට බැස්සුවේ බයික් එකෙන්.ආයේ හවසට දුවව අරන් 3 වෙනකම් හොස්පිටල් එක ලග ඉදලා වයිෆ්වත් අරන් ගෙදර ආවෙ ට්රාන්ස්පෝර්ට් වලට යන වියදම් අඩු කරන්න ට්රාන්ස්පෝර්ට් වලට යන කාලය අඩු කරන්න.දෙන්නා බස් එකේ වැඩට සහ පාසල් ගියොත් මාසෙට ට්රාන්ස්පෝර්ට් වියදම 25000ට වැඩියි. දැන් තෙල් හොයාගන්න බැරිම තැන දුවගේ පාසල් ගමන ගිය සිකුරාදා ඉදලා නැවත්තුවා. වයිෆ් දැනට බස් එකේ යනවා ඒකත් වැඩි කලක් කරන්න හම්බෙන්නේ නෑ.සෞඛ්ය අංශේ සේවකයෝ ලොකු කැපකිරීම් කරලා සේවය සපයන්නේ. ලොක්ඩවුන් කාලවල අනිත් අය සැපට ඉදන් පඩි ගනිද්දිත් තමන්ගෙ වාහනවලින් ගිහින් සේවය සැපයුවා.ඒ මිනිස්සුන්ගෙත් ඉවසීම ඉවරවේගෙන යන්නේ.මේ අවුරුද්ද අන්තිම වෙනකොට සෞඛ්ය සේවය කඩා වැටීම නවත්වන්න බෑ. පුලුවන් සියලුම ඩොක්ටර්ස්ලා , නර්ස්ලා මයිග්රේට් වෙන්න හදන්නේ.දැනට ලංකාවෙ ඉන්න නර්ස්ලා 40000න් 20000ක් වත් ඉතුරු වෙන එකක් නෑ.
