1. Get a photo from certified studio and obtain the photo aknowledgment reference printout - http://www.immigration.gov.lk/web/i...ontent&view=article&id=302&lang=en&Itemid=190
2. Make an online appointment - https://eservices.immigration.gov.lk/appointment/pages/reservationApplication.xhtml
3. Download and fill the passport application - http://www.immigration.gov.lk/web/images/stories/pdf/form/passport/passport_application.pdf
4. Make sure you have below documents as required.
What are the documents required to be submitted along with the completed application form ?

1. Current passport with a photocopy of the Bio data page. (Please see below for details*)
2. Photo studio acknowledgement
3. Original Birth Certificate of the applicant with a photocopy.
4. Original National Identity Card of the applicant with a photocopy
5. Marriage certificate with a photocopy where it is necessary (To confirm the name after marriage)
6. Educational Certificate related to the profession and an acceptable document to confirm your service and photocopies thereof.

• On the day which you get the appointment date/time, be at the passport office 30-45 mins prior your given time.
• Go to the reception and mark your attendance and they will give you a token number (they will check and make a signature on your photo acknowledgement/appointment)
• Go to the 2nd floor to confirm your documents.
• Make sure you staple/clip all documents (ask them to do they will point you to a place which does that for you)
• Then you will be asked to go to the application processing place (2nd floor)
• Once they verify all documents are in place, you will be asked to get fingerprints taken and then make the payments

You can get all done in 30-40 mins tops.

Seems like all days are booked until August 19th. Keep on checking all days if it shows a free slot.

If you don't put an appointment and visit, there will be a bloody long queue and the chance of getting in is absolutely not sure.
