Usually they are made in an in-house cyclotron. Also there are different types of tracers - depending on what you are trying to diagnose.
Also there are FDG and Rb82, Zr89 based tracers that do have a longer half life. Surely the doctors will have some plan on how to source the tracers. Otherwise how can you the scanner!
