TO OBTAIN A DEATH CERTIFICATE.. WHAT DOCUMENTS REQUIRED???

    To obtain registered death certificate of my father what do you need?

    What original documents need to be submitted.????

    Any copies of originals required to submitted????

    Any signatures required from Grama Niladhari????

    Any certifications of copies required???

    How many copied required to submit???

    PLEASE HELP IF U CAN, MUCH APPRECIATED. THANK YOU

    1. Refer the application to the Divisional Secretariat of the area where the death is occurred.
    2. Application can be obtained from any Divisional Secretariat.
    3. Fees chargeable
      1. Where the date of registration or the no. of the entry is given the fee for one copy of the certificate is Rs. 100.00
      2. Where the date of registration or the no. of the entry is not given and a search of registers not exceeding two years is involved fee for one copy of the certificate is Rs. 200.00
    1. Required fee should pay to the Divisional Secretariat.
    2. Duly filled application should submit to the Divisional Secretariat.
    3. If you like to obtain the certificate of death by post, submit a stamped envelop with the application.
    4. If the death is registered a certificate of death will be send and if the death is not registered a letter to the effect will be send.
