Nowadays you need to get a new passport for the child as well, you can either apply for 3 years or for 5 years. but it is suggested that you get it for 3 years.

Are you sure of this?? Or just guessing? I don't thing the Department of I&E is stupid to do this.

AFAIK you submit Form 35C along with parent's passports (One parent plus a letter of authority from the other) and of course the Child's Birth Certificate.
Upto 6 children can be included.

all those forms are outdated, it happened to me for my 1 year old child. i had to take separate passport for him. i am sure this is the case for all. if i am correctly recalling the number you should be able to contact 1919 and get it confirmed.

Then of course it should be correct from your own personal experience. But certainly it is a bloody stupid move. (not thinking of the considerable monetary gain for the Dept of I&E with the issue of a new passport for every infant)
