Softlayer.com - Probably the most influential in US. Wondering where is ThePlanet? Well they bough it without consideration. Big company with good reputation. Most of major Webhosting Companies are using their facilities. Good quality. High price, but you know what you get.
Hostgator.com - Everybody knows Hostgator. They offer dedicated servers as well. They use Softlayer.com facilities, so basically you are getting Hostgator support with Softlayers servers. A bit pricey, but trusted solution.
100TB.com - I cant really find bad review about them. Good and powerful servers. 100 TB (!!!) bandwidth with each of them. Price is sky high, but it is worth if you need loadz of resources.
Gigenet.com - Dedicated server provider that offers DDOS protection. Good service and reasonable plans, but they will try to upsell you on any possible occasion.
Rackspace.com - Good quality but prices of servers are crazy. Not worth the money. Left them for big companies.
Liquidweb.com - Again, sky high prices. They are dominating market thanks to the fact, that they host largest hosting forum in web.Not worth the money.
Staminus.com - So many problems I had by them. They are bulletproof US based provider (copy of McColo). They allow all type of things - Porn, Child porn, hate etc... But will never really approve anything. So you can abuse their servers as long as you will not ask stupid question "can I do this?". Prices are good, but the way they run business is crazy. Major downtimes, trying to sell you additional services in every occasion. Understaffed and with 24 hours ticket response time.
Yahoo.com - Just don't use them. So many bad reviews and unhappy customers... Simply ignore them all the way.
WiredTree.com - 7 days downtime? With Wired Tree everything is possible! Avoid them! Many webhosting companies that used their servers went bankrupt because of downtime.
Singlehop.com - Another "No Uptime" hosting provider. They offer fully managed servers and minimum (???) support. Lately they just disappeared without explanation. After 3 days they showed up blaming DDOS attack... C'mon, thousands servers and whole DC was hijacked by DDOS? Sure it was.
Canada:
Rackforce.com - Not so many reviews, but they are around for 5 years, their prices are really good, and they have good location in British Columbia, Vancouver - next to intercontinental fiber to Asia.
UK:
Webfusion.co.uk - bad reviews, army of unhappy customers. Waste of time really.
UK has so many low quality hosting companies so I will simply leave this section until I will find quality provider.
France:
OVH.com - why this one is in red? Good servers, crazy low prices, amazing resources... Yeah, sounds great. But cons are so big that I am asking you to avoid them as for now. Poor customer support is key point to not going with them (if you don't speak French you will be ignored and treated like crap).
For some reason they hate Americans too, so if you want their servers, dream on...
Netherlands:
Leaseweb.com - Best around. One of the best companies on the globe. By now they have something like 20.000 servers under management. Good prices, greate uptime and dedicated network. Single company that made from Netherlands largest European network provider.
Ecatel.nl - Truly bulletproof solution for dedicated hosting. They officially don't exist. And yes, they don't even have a website.Reasonable prices and good network. But support is not the best. They improved lately, but if you don't plan to hire support team or technicians, this is not place for you.
Netrouting.nl - Good prices for servers and VPS. But absolutely NO support. If you want to get support you need to pay up to €300 a month! If you will decide to go with basic support prepare yourself for 20-24 hours response time - every time. Their Sales department do not answer to most of the tickets too, which is really bad. They have good network and small lags. Not recommended if you dont have your own support/technician team.
Prefiber.nl - Cheapest around. VPS from €1.95 per month! Its cool place if 95% uptime is what you are looking for! Lost data? Happens. 3 days downtime? Happens. 24/7 support? They say they have, but dont count on it. Weekend open? Nope, they do not work weekends. Typical company run by 1 guy with help of 2 students that know nothing about hosting. Avoid it!
Germany:
Hosteurope.de - Best choice for Germany really. Good support (they even try to speak English!), amazing hardware, and great plans. Don't expect to much from support, but they respond, and thats great. One of best choices for European market.
Netdirekt.de - Great hosting company. Recently bought by Leaseweb, so it is much better to go to the source really...
Hetzner.de - Good company but a bit ignorant. Great prices, and hardware. Do not expect much from support. They will respond but their knowledge of English (even on English support line) is close to zero. Many scary stories around, but overall good company.
Webtropia.de - Good software, great network and prices. But they will ignore most of the support tickets, they require 1 year contract too. Not recommended at all.
Poland:

Sweden:
PRQ.se - Owned by ThePirateBay guys. High prices, but they will cover your ass whatever is the crime you are doing. Highly reliable and reasonable place to have Black Hat server. Ordering is bit tricky, but it is worth it.
Ukraine:
JustHost.in.ua - Fromall hosts in Ukraine this looks the best. They actually accept Paypal, do not care about copyright and anything else really... Prices are great and you get 100 Mbps unlimited connection. I wont post rest of the hosts as they will be in red for difficulty with support, payment methods and lack of English support.
Russia:
2x4.ru - Many good reviews and many bad ones. Good support, but no English version of site (just Google Translate). Prices are really high for what you get. High pings and lags. But if you need something remote, then thats the place for you.
Malaysia:
Shinjiru.com - Hard to find more remote place to host your BH stuff. Good english support, good prices. But connection is still behind any norms. 1 Mbps for dedicated server?? C'mon!!!
DS.my - Cheap dedicated server provider. It is giving 100 Mbps for all servers, but says nothing about what is real connection you can get from it. Poor customer support. Live chat is most of the time unattended.
Egypt:
Citynethost.com - Some informations about downtimes floats around the web, but it is a good and cheap company. They don't really care about copyrighted material, which is cool. But remember - Egypt is No-Porn country!
