Which countries refuse if you carry both passports - with the visa stamped on the expired one? In that case any long term residence permits can be re-evidenced onto the new passport.

Wasn't for me. Long-term residence permits in EU are physical cards similar to ID cards. Some contain the passport number
Yeah, it is quite an easy process to transfer the visa / residence permit from the old passport to the new one.
