tiling on an old wall

when tiling on an old wall (existing) -
a. chip off existing surface
b. apply cement grout on the surface (how to maintain vertical level?)
c. fix tiles on grouted surface

my question is if I didnt maintain vertical level of grouted surface, then the finished surface after tiling also become uneven.

please advice how to do this ?
