මම මේ ගැන පොඩ්ඩක් කියන්න හිතුවෙ, clinic වලට අලුතෙන් එන ගොඩක් රෝගීන් ගෙ තියෙන අඩුපාඩුවක් දැකල.

මේක බලන asthma තියෙන කීදෙනෙක් salbutamol (සැල්බ්යුටමොල්) එහෙමත් නැත්නම් නිල් පාට බෝතලයෙ එන ගෑස් එක, එහෙමත් නැත්නම් නිල් පාට කරල්, එත් නැත්නම් ෆාමසියෙන් හතියට ගන්න රොස පාට පොඩි පෙති "විතරක්" ගන්නවද?
එහෙම ගන්නවා නම්
පොඩ්ඩක් එ ගැන සදහන් කරන්න.

මේ salbutamol භාවිතය ගත්තොත් ගොඩක් අය එවෙලෙට ලැබෙන සහනය නිසා පාවිච්චි කරනවා. නමුත් ඒක විතරක් භවිතය ගොඩක් අහිතකරයි වගෙම, රෝගය හරියට පාලනය වෙන්නෙ නැහැ. එන්න එන්නම රෝගය වැඩි වෙනවා. රෝගය පාලනය වෙන්නෙ නැහැ කියන්නෙ, පෙනහලු වල permanant වෙනස්කම් වෙන්න ගන්නවා. එතකොට මොන බෙහෙතට වත් පාලනය කරන්න අමරු වෙනව වගෙම, එක පාරටම ගොඩක් දරුනු attack එකක් එන්නත් පුලුවන් සහ, එම නිසා ජීවිත හානි වුනත් එන්න පුලුවන්.
එ වගේම salbutamol වලට අපෙ ඇග පුරුදු වෙලා බෙහෙත අහන්නෙ නැතුව යන්න ගන්නවා.

මේ වගෙ හේතු ගොඩක් නිසා salbutamol විතරක් ගන්න එක නොකල යුත්තක්.

නමුත් ක්ලිනික් වලට අලුතෙන් එන, සහ හරියට රෝගය පලනය නොවු ගොඩක් අයගෙ දකින්න පුලුවන් දෙයක් තමයි මේ වැරැද්ද.
ඒක නිසා, මම මේ දැනුවත් කිරිම කරන්නෙ, තමුන් හෝ, ගෙදර කවුරු හරි හෝ, මේ වැරැද්ද්ද කරනවා නම් ඔවුන් ව දැනුවත් කරන්න කියන්න.

(වරදවා වටහා ගන්න එපා, මෙතන මම කියන්නෙ salbutamol නරක බෙහෙතක් වත්, ඒකෙන් මේ හානි කරනවා කියලා වත් නෙවෙයි, හරි ප්‍රතිකාරය නොලැබීම නිසා මෙම හානි වෙනවා කියන එක. Salbutamol හරි salmetrol හරි inhaled steroid එකක් එක්ක ගන්න අය බය වෙන්න එපා, ඒ අය හරි බෙහෙත තමයි පවිච්චි කරන්නේ (asthma කියන ලෙඩේ හරියට diagnose කරල නම්))

salbutamol වලින් නම් එහෙම දෙයක් වෙන්නේ නැහැ පොඩි කාලේ ඉදලා බොනවා. ඕක බිව්වා ම වෙන්නේ සෙම තද වෙන වෙලාවට සේම තද වෙන තැන් විශාල වෙලා හුස්ම ගන්න ලේසි වෙන එක. ඕක බිලා ලෙඩේනම් හොද වෙන්නේ නැහැ නමුත් සේම තද වෙන වෙලාවට ඕක මාර සහනයක්. ඕක බිව්වා ම ගැහෙන්න ගන්නවා මාර විදියට

මටයි අම්මාටයි හොදටම තිබ්බා අවුරුදු 20කට කලින් ඇදුම පොඩි කාලේ අමාරු වෙලා Hospitel අරගෙන යනවා මාව nebulizer කරලා තමා අඩු වෙන්නේ වැඩි උණාම. ඇදුම ලෙඩේට බෙහෙත් කරන specialist Docter කෙනක් හිටියා ඒ කාලේ එයා ලංකාවේ හොදම එක්කෙනා කියලා කිව්වේ එයාගෙන් අපි බෙහෙත් ගත්තා මාස 6ක් විතර එයා දුන්නේ Deflapin / prednisolone විතරයි හොද උණා අපිට හොදටම. ඊට පස්සේ ඉදලා හිටලා සේම තද උනොත් විතරක් salbutamol බොනවා දවස ගානේ බොන්නේ නැහැ දැන් හැබැයි

I had written on this before. You know that asthma cannot really be cured but managed. That's why you need to stick with one consultant. How they decide is by Spirometry. Over a period he should determine which drug regime because the disease severity, the effective management with a particular drug can then be assessed and changed.

Seroflo contains Fluticasone Propionate and Salmeterol.
Foracort contains Formoterol and Budesonide.

There are a lot of medications available but they all should be taken on the advice of the physician. Recently I received some samples while on an overseas trip of a new FDA approved drug Breztri Aerosphere which contains three components. These samples have only 28 doses each - (retail version in the US is about $350 or so) and gave these to a friend to trial on a few COPD sufferers. Could be available in India.

Hmm, sounds like something a well-informed patient could do by self start with the lowest dose of Seroflo or Foracort and test both over a few months to see which medication works best.

I have experience with both medications and what worked best for me was losing weight if you can believe it Went from an obese BMI to a normal BMI over three years and the frequency of my asthma attacks reduced to about once a month so I stopped using my preventer inhaler, still have to use the reliever if exposed to heavy dust or if I come down with some other illness like viral flu though

Yes they are related....Obesity raises the risk of asthma (BMI > 30) and also asthma raises the risk of developing obesity.
