From my understanding, majority of the big names out there started off with foreign clients and then maybe moved onto local customers as well.
No, I don’t freelance but I do know a company founder of such a service company and there is a “sudda” involved in the genesis story.
And then there are the smaller companies that started off with Upwork clients or as breakaway factions from larger companies - mostly with foreign clients.

I honestly think it’s easier to find foreign clients than going after local clients in this day and age.All I know is I am paying £50 per month for a bloody accounting software. They are not even market leaders but they rapidly grow in to 3mil subscribers last few years.
In another note, a friend of mine who lives in SL is earning more than 50k USD annually. I've never heard of his Norwegian company but they seems to doing well. They dont do anything with the locals customers.
